Length 4 to 5 mm. Cicadula quadrinotata is a yellow-green leafhopper with black spots on the vertex and rather long antennae. Dissection of males is required for definite identification.

Females cannot be distinguished from similar species. Males can be identified by microscopic examination of genitalia. 

In damp grasslands where it is associated with rushes and sedges.
